New Patients
Whether you are expecting, have a new baby, have had a change of insurance, are new to the area, or are searching for a new pediatrician, we would like to welcome you to our practice.
First Visit
Whether you are bringing in your newborn for his/her first visit or are transferring, please complete our Registration Packet prior to the visit. Bring all forms plus your drivers’ license and insurance card with you to the first visit. This will help you get through the check in process as quickly as possible.
New babies
Congratulation on your new arrival! We will gladly take over the care from babies born at all area hospitals and birthing centers, you do not need to register with our practice before your baby is born.
When your baby is born, the pediatrician on call at the hospital you give birth will care for you while in hospital. Please call us at discharge to schedule your newborns first visit. We will want to see you in our office 24-48hours after discharge to check your babys weight and address any concerns, including jaundice. Please request a copy your childs Newborn discharge records to take with you at discharge and bring to your first appointment.
